Family Case Managment

Pregnant women and families with a child under one year old who live in Kane County and are below 200% of the Federal Poverty Level qualify for this program. VNA family case managers can provide referrals for both medical and social services. Women do not need to be a VNA Health Care patient to use Family Case Management services. These services are integrated with those provided through the Women, Infant and Children (WIC) nutrition program. VNA’s Family Case Management program is located within the WIC department at our Aurora/Highland Ave. location.
