Transportation Assistance
If you do not have a ride, transportation assistance options are available to and from our clinics. Please ask our staff for details or call us for help arranging a ride. For some patients, it may be more beneficial to receive care in their homes, including pregnant women, those with certain disabilities or those newly post-surgery. Ask our staff about VNA’s Home Based Care. If you aren’t sure how to get started, call us for help finding transportation options.
Requesting Your Medical Record
VNA protects patient health information. For this reason, we need you to sign and submit an Authorization for Release of Patient Health Information form to obtain a copy of your medical record or to authorize VNA to release your medical records to a third party.
You can download a virtual copy of your medical records for free using MyVNA. If you want a hard copy of your medical records, we might charge you a small photocopying fee.

Insurance, Payment and Billing Information
The cost of your VNA appointment depends on several factors. Many patients obtain full coverage through their insurance plans. Some patients only obtain partial coverage from their insurance. However, VNA offers discounts for patients based on family size and income, as well as payment plans. We also provide assistance applying for benefits. If you are under-insured or uninsured and you qualify, we can help you get coverage at a lower cost than elsewhere. We work hard to offer you quality services at the most affordable price. No one will be turned away for inability to pay.
VNA Health Care accepts Medicare, Medicaid and many commercial insurance plans. We are constantly working to contract with new plans. When we add new plans, we update our insurance page. Visit the page to see currently accepted insurance plans.
All patients qualify to receive help from VNA benefits assistance to determine if they qualify for benefits or programs. We encourage all new patients to sign up for benefits assistance. If you don’t have insurance, or if your insurance doesn’t cover your entire bill, VNA Health Care offers payment plans to make it easier for you to access care. We also offer discounts based on family size and income. Remember that our staff can also help you identify benefits programs and determine whether you qualify.
