Your Medical Records Track Your Healthcare Journey at VNA

VNA Health Care adheres to policies that protect your information while giving you and other parties you designate access to information when it is needed the most. When you choose to sign up for MyVNA, you will be able to request and share your vital medical information securely online whenever and wherever it is needed.

How to request your medical records

To obtain medicals records or to authorize VNA to release it to another party, you must complete our Authorization for Release of Patient Health Information. After completing the form, fax it to VNA at (630) 482-8180 or mail it to 400 N. Highland Avenue, Aurora, IL 60506. You may also drop it off at the VNA Health Care located at 400 N Highland Avenue, Aurora, IL 60506.

How long is a consent form good for?

Expiration of consent is 1 year for a general Medical Records request. If you are requesting Behavioral Health records, the expiration is 90 days. Once your consent form is past its expiration date, you will need to fill out and return a new consent form.

Can I get my Imaging records at VNA?

VNA only provides imaging records for imaging related to the Illinois Breast and Cervical Cancer Program at this time.

How much does a hard-copy of a medical record cost?

Any photocopying of your record may incur a small fee. If picking up the record in person, you may pay the fee upon pickup at VNA Health Care, 400 N Highland Avenue, Aurora, IL 60506.

10-49 pages – $10
50+ pages – $20

Our policy states your medical record will be sent within 30 days of request; however, most requests arrive much sooner.

Accessing a medical record safely and securely online


Good news, if your visit occurred after October 2022, you can now login into MyVNA to access or share you VNA Health Care medical record at your convenience.

There is no fee for requesting a medical record through MyVNA.

MyVNA will allow you to share your patient record with other health care providers or download it for your own records at no charge.

How can I sign up for MyVNA?

Get Your Code – Call (630) 892-4355 or ask your care team for the code.

Visit MyVNA to set up your account.

Once I have signed in to MyVNA where can I find my Medical Record?

After logging in to MyVNA, navigate to the Your Menu icon in the top left corner.

Select Sharing Hub under the Sharing Menu.

On the Sharing Hub page, select who you want to share your health record withYou can choose to send your medical record to yourself or to someone else.

Then select Request a formal copy to obtain your full medical record to share with your providerYou can also choose to share access to your MyVNA account or to download an informal copy.

If you have sent the record to yourself, search Document Center from the search menu. Once the request is processed, you will be able to download your record.
