In the MyVNA patient portal you can schedule an appointment, view and request patient medical records, referrals, prescription refills, and more.

To obtain your medical record or authorize VNA to release it to another party, you must complete our Authorization for Release of Patient Health Information.

VNA offers School Based Health Centers at East Aurora High School and Romeoville High School. VNA also collaborates with other local school districts. In order to share student medical records with their school district, students under 18 must fill out and return the consent formed signed by their parent or legal guardian.
