Before Your Appointment

  1. Register at the MyVNA Patient Portal. Follow our tutorial to learn how.
  2. You need to complete and submit a consent form before your appointment. Completing and submitting the form online will make check-in faster. Alternatively, you can complete the form at the health center when you arrive.
  3. Bring your photo ID and insurance card to your appointment. You will be notified if any payment is due.
  4. If available, bring a copy of any past test results, imaging studies, immunization records, or medical records related to your current concern.
  5. Bring a complete list of your current medications with dosages to your appointment. This includes prescriptions, over-the-counter medications and supplements.
  6. If you are interested in qualifying for benefits, click here to view the items to bring and how to request benefits assistance from our team. You can request assistance before your appointment to save time. Otherwise, you can request assistance when you arrive for your first appointment.
  7. Remember to arrive at your appointment 15 minutes early to allow time to check in at the registration desk.
  8. If you are already a VNA patient and need a copy of your medical record, please complete and sign the consent form to release your information.

During Your Appointment

  1. You will initially wait in the waiting area. A member of our clinical team will take you to an exam room, once it is ready.
  2. If you require language assistance, inform our clinical team. Our staff is bilingual in English and Spanish, and we use a translation service in the exam room for hundreds of languages.
  3. Most exams will start with a review of the patient’s health history, a discussion of the purpose of the visit and any current health concerns, and a taking of vitals.
  4. Your VNA provider will conduct the appropriate physical exam, ask questions, and provide diagnoses, recommendations, and/or next steps, as appropriate. They may order labs, screenings, vaccinations, prescriptions, or other follow-up care.
  5. You will receive an after-visit summary, which will include your follow-up plan and next steps – such as test orders, prescriptions, and referrals to specialty care.
  6. You will book your next appointment before you leave the clinic, either in the exam room or when you check out.

After Your Appointment

  1. You may go to the lab before you leave the VNA clinic to get your blood drawn for tests ordered by your VNA provider.
  2. You may need to fill a prescription. You can stop at one of VNA’s in-clinic pharmacies (in Aurora, Elgin, and Joliet) or head to one of the hundreds of retail and specialty pharmacies VNA partners with to help qualifying patients receive 340b prescription discounts.   3. You may need to set up follow-up appointments, either with VNA (if you didn’t schedule them before you left) or through a referral that was provided to you. To schedule an appointment with a referred specialist, review this information about referrals.
  4. You may request a copy of your medical record via MyVNA.
