At VNA you receive assistance in helping to prevent and manage your chronic condition from a comprehensive team of healthcare professionals including doctors, nurse practitioners, CNA’s, registered nurses, pharmacists, dietitians, and nutritionists.
According to the Centers for Disease Control and Prevention, chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both. Chronic diseases such as heart disease, cancer, and diabetes are the leading causes of death and disability in the United States.
Many chronic diseases are caused by a short list of lifestyle choices:
- Tobacco use and exposure to secondhand smoke.
- Poor nutrition, including diets low in fruits and vegetables and high in sodium and saturated fats.
- Lack of physical activity.
- Excessive alcohol use.
VNA Can Help!
VNA offers free wellness classes where participants learn more about preventing and managing their chronic diseases, including diabetes. Evidence-based classes are fun and interactive and help participants learn more about how to cook culturally familiar foods in a more healthy way. This is a great way to learn more and meet others who are facing similar challenges and are improving their health.
We have resources for your Chronic Disease Management & Prevention at VNA Health Care.
