VNA Outreach Representatives are available to share health education, presentations and resources at community events and locations.

They are knowledgeable, friendly and a great resource for individuals and community groups. If you would like to invite a VNA Outreach Representative/Community Health Worker to talk with your clients or participate in a community event, please reach out to us. Presentations and materials can be delivered/provided in either English or Spanish.
Our goal is to help navigate those who need it the most to the care services they need to optimize their health and social needs.
