If you are a VNA patient who has been diagnosed with diabetes, you are eligible to join us for this one-stop clinic visit. In just one convenient visit, you can get:
- A1c check (plus any other primary care checks you need, ncluding blood pressure)
- Eye health screening (via retinal imaging)
- Foot exam
- Medication review
- Diabetic education
- Prescription refills
- Appointments made for nutritional counseling, clinical pharmacy & other care

Type 1 Diabetes is a condition where the body cannot make insulin. Insulin is a hormone that helps sugar (glucose) move from the blood into the body’s cells for energy. In Type 1 diabetes, the immune system attacks the cells in the pancreas that make insulin. Because of this, people with Type 1 diabetes need to take insulin every day to keep their blood sugar at a healthy level.
Pre-diabetes means your blood sugar levels are higher than normal but not high enough to be diabetes. It is a warning sign that you may develop Type 2 diabetes in the future. The good news is that healthy changes—like eating well, being active, and maintaining a healthy weight—can help prevent or delay diabetes.
Gestational Diabetes is a type of diabetes that develops during pregnancy. It happens when the body cannot make enough insulin to keep blood sugar at a healthy level while pregnant. Gestational diabetes usually goes away after the baby is born, but it means the mother may have a higher chance of developing Type 2 diabetes later in life.
Type 2 Diabetes is a condition where the body does not use insulin properly or does not make enough insulin. When insulin does not work well, sugar builds up in the blood. Over time, high blood sugar can cause health problems. Type 2 diabetes can often be managed with healthy eating, physical activity, medications, and sometimes insulin.
