If you are a VNA patient who has been diagnosed with diabetes, you are eligible to join us for this one-stop clinic visit. In just one convenient visit, you can get:

  • A1c check (plus any other primary care checks you need, ncluding blood pressure)
  • Eye health screening (via retinal imaging)
  • Foot exam
  • Medication review
  • Diabetic education
  • Prescription refills
  • Appointments made for nutritional counseling, clinical pharmacy & other care

Diabetes Healthcare
VNA primary care providers are board-certified in Family Practice/Internal Medicine, and care for thousands of patients per year with Diabetes. Our services include:

  • Podiatry: Foot exams and treatment to prevent infections, ulcers, and other complications
  • Psychiatry and counseling: to help you cope with the emotional challenges of living with diabetes

Diabetes Medications
Managing diabetes often requires medication, and VNA makes it easy and affordable to get the prescriptions you need and the required follow-up care to make treatment effective. We offer:
  • Prescriptions: Your provider will prescribe the right medications to control your blood sugar and prevent complications
  • Regular medication reviews: Our clinical pharmacists work with you to make sure your medications are effective, safe, and affordable
  • On-site pharmacies: Convenient pharmacies at select VNA locations so you can fill your prescriptions before you leave
  • Drug discounts through the 340b Program: Significant savings on medications for eligible patients
We partner with local Walgreens, CVS, and Walmart pharmacies to extend these discounts to patients who prefer to fill prescriptions closer to home. We also partner with specialty and mail order pharmacies.

Types of Diabetes We Treat
Each type of diabetes is different. We tailor our treatment plans to meet your specific needs. At VNA, we offer treatment for:
Type 1 Diabetes
Type 1 Diabetes is a condition where the body cannot make insulin. Insulin is a hormone that helps sugar (glucose) move from the blood into the body’s cells for energy. In Type 1 diabetes, the immune system attacks the cells in the pancreas that make insulin. Because of this, people with Type 1 diabetes need to take insulin every day to keep their blood sugar at a healthy level.
Pre-Diabetes
Pre-diabetes means your blood sugar levels are higher than normal but not high enough to be diabetes. It is a warning sign that you may develop Type 2 diabetes in the future. The good news is that healthy changes—like eating well, being active, and maintaining a healthy weight—can help prevent or delay diabetes.
Gestational Diabetes
Gestational Diabetes is a type of diabetes that develops during pregnancy. It happens when the body cannot make enough insulin to keep blood sugar at a healthy level while pregnant. Gestational diabetes usually goes away after the baby is born, but it means the mother may have a higher chance of developing Type 2 diabetes later in life.
Type 2 Diabetes
Type 2 Diabetes is a condition where the body does not use insulin properly or does not make enough insulin. When insulin does not work well, sugar builds up in the blood. Over time, high blood sugar can cause health problems. Type 2 diabetes can often be managed with healthy eating, physical activity, medications, and sometimes insulin.
