What is Home Health Care?

Home health care is professional medical care provided in your home by licensed healthcare providers. It’s designed for individuals who need skilled nursing, therapy, or personal care but don’t require hospitalization. VNA has been providing care in the home since our founding in 1918.

Home health care is usually more convenient, less expensive and just as effective as the care you can get in a hospital or nursing home. For some people, receiving care at home can be the best temporary or permanent option.

Who Qualifies for Home Health Care Services?

Home health care is appropriate for a wide range of patients, including:

  • Individuals recovering from surgery, illness, or hospitalization
  • Patients managing chronic conditions such as diabetes, heart disease, or COPD
  • Seniors who need assistance with daily activities and medication management
  • People with mobility limitations who benefit from physical or occupational therapy at home

To qualify for home health care services, you typically need to be homebound (meaning leaving home requires considerable effort), under the care of a physician, and in need of skilled nursing or therapy services. How Does Home Health Care Work?

VNA Home Health Care provides you with a Registered Nurse (RN) or Certified Nurse Assistant (CNA) to:

  • Make regular visits to your home to provide care
  • Coordinate the services you need from other healthcare providers, including doctors, therapists, social workers and nurse aides
  • Keep you, your family and your doctor informed about your progress and care plan

More Home-Based Services at VNA

If you don’t need home health care, but it’s difficult for you to leave the house for a doctor’s appointment, you can try VNA House Calls. We send a doctor or Nurse Practitioner to your home to provide the same care you would receive at the center. VNA also offers home-based hospice services for patients who need end of life care.

Home Health Care and Insurance

VNA Healthcare accepts a wide range of insurance plans and our team works with you to maximize your benefits. Our benefits assistance team can help you navigate coverage questions and apply for programs you may qualify for.

Many insurance plans cover home health care services when they are medically necessary and ordered by a doctor. The extent of coverage depends on your specific insurance plan and medical needs.

VNA Health Care accepts most major insurance plans. We also work with patients who have limited or no insurance, offering a sliding-fee scale based on income and family size to ensure everyone can access the care they need.

Yes, Medicare Part A and Part B cover home health care services if you are homebound and have a doctor’s order. Medicare covers 100% of the cost for medically necessary home health visits. There is no deductible or copayment for Medicare-covered home health services. VNA Health Care is a Medicare-certified provider, and our team will work with you to ensure all necessary documentation is in place for coverage.

Yes, Medicaid covers home health care services in Illinois when they are medically necessary and prescribed by a doctor. VNA Health Care accepts Medicaid and can help you understand your benefits. If you’re not currently enrolled in Medicaid but think you may qualify, our benefits assistance team can help you apply.
